〜花粉症の季節ですねぇ。It is allergy season.〜

「年々花粉症が悪化している気がする」と思っているあなた。気のせいではないんです！2021年にPNAS (Proceedings of the National Academy of Sciences)に発表された研究結果によると、過去30年で花粉症の季節が20日早く始まり、8日間長くなったそうです。また、花粉の量や飛散濃度も21％増加。気候変動で二酸化炭素と気温が上昇すると植物が生産する花粉の量が増えますが、2022年のNature Communicationsに発表された論文によると、2100年における気温の上昇が摂氏2-3度の場合と4-6度の場合を比べると、花粉量のシミュレーション結果は気温の上昇が4-6度の場合は2-3度の場合の2倍になるそうです。

If you feel like “my seasonal allergy is getting worse every year”, you’re not just imagining it! According to a study published in PNAS (Proceedings of the National Academy of Sciences) in 2021, the allergy season has
started 20 days earlier and gotten 8 days longer over the past 30 years. Pollen count and pollen concentration also increased by 21%. As carbon dioxide and temperatures rise due to climate change, the amount of pollen
produced by plants increases. According to a paper published in Nature Communications in 2022, comparing the cases in 2100 where the temperature rise is 2-3 degrees Celsius with the cases where it is 4-6 degrees Celsius, the simulated pollen count is said to be twice as high when the temperature rise is 4-6 degrees Celsius compared to when it is 2-3 degrees Celsius.

花粉症の原因は花粉の量だけではありません。環境汚染物質が花粉と結合したり花粉を分解することで花粉の構造が変わり、アレルゲンとなる可能性が高くなります。

Seasonal allergy is not just caused by the amount of pollen. Environmental pollutants can bind to or break down pollen, changing the structure of the pollen and making it more likely to become an allergen.

新たに花粉症になる方もいるでしょうね。わたしの勤めているクリニックでも、毎日花粉症薬の処方箋を受け取っています。もうそろそろピークシーズンになるので在庫を増やさないと💦 ただの花粉症だったらまだいいんですが、かわいそうなのが喘息持ちの患者さん。花粉が増えると喘息が悪化するので、Urgent Careや救急に駆け込む患者さんが増えるんです。死活問題ですね。

Some people may have newly developed seasonal allergy. At the clinic where I work, we receive prescriptions for allergy medicine every day. It’s almost peak season, so we need to stock up allergy meds💦 If it were just seasonal allergy, that would be fine, but the poor asthma patients are the ones who suffer more. When pollen levels increase, asthma gets worse, and more patients end up in urgent care or emergency room. It’s a matter of life or death.

わたしが住んでいるオレゴン州ポートランド市でも、越してきた20年前から花粉症の患者さんはいっぱいいます。「5年も住んでいればみんな花粉症になるよ」と言っていた人がいたほど。わたしもポートランドに住んで3年で花粉症になりました。さて、そのわたしが花粉症の季節に外に行くか行かないか決めるのに使っているのがPollen.com。今日の花粉飛散量、5日間の予報、過去30日間の記録、さらに何の花粉が飛んでいるのかがわかります。例えば今日は7.9 (中ー高)で草類、樫、くるみの花粉が飛んでいるそうです。花粉の飛散量は0から12まで（昔は10までだったと思うんだけどなぁ）のスケールで表示されます。週末は庭の手入れが滞っていたので、ほぼ一日中外にいたんですが、昼からはくしゃみが止まらず、目も痒くて仕方ありませんでした。

In Portland, Oregon, where I live, there have been many people who have suffered from seasonal allergy since I moved here 20 years ago. Someone once said, “If you live here for five years, everyone will get seasonal allergy.” I had been living in Portland for three years when I developed seasonal allergy. Now, I use Pollen.com to decide whether or not to go outside during allergy season. It shows today’s pollen count, the forecast for the next five days, the history of the last 30 days, and what kind of pollen is in the air. For example, today it was 6.1 (medium), and it says that grasses, oak, and walnut pollen are in the air. The pollen count is displayed on a scale of
0 to 12 (I think it used to be 10). I was behind on gardening until this past weekend, so I was outside almost all day last weekend. I couldn’t stop
sneezing and my eyes were itchy in the afternoon.

せっかくですので、たまには薬剤師らしく薬のことも書こうと思います。

Since I have the opportunity, I thought I’d write about medicines as a pharmacist would.

OTC (Over-the-Counter)：処方箋なしで買える薬：Drugs that can be purchased without a prescription

最近はほとんどのアレルギー薬が処方箋なしで薬局やスーパーで買うことができます。逆にどれを買っていいか迷うほど。どれがいいかは、値段と好み、効きやすさで人それぞれなので、いろいろ試してみるといいかと思います。

Nowadays, most allergy medications can be purchased at pharmacies and grocery stores without a prescription. It can be hard to decide which one to buy. The best one depends on the price, preference, and effectiveness, so it’s a good idea to try out a few different ones.

服用剤は体のあちこちに症状が出る人に最適。第二世代の抗ヒスタミン剤は眠くならないと言って売られていますが、敏感な人はやはり眠くなってしまうので、運転に気をつけないといけません。また、頭痛や口腔が乾燥するなどの副作用があります。第一世代の抗ヒスタミン剤のベナドリル。とってもよく効きますが、すごく眠くなります。睡眠薬として使っている人もいます。また、口腔や目が乾燥したり、混乱などの副作用もあります。第二世代の抗ヒスタミン剤の効きが悪い時は、ベナドリルを屯用で併用処方する医者もいます。psudoephedrine（鼻づまり薬）の入っている薬は棚には置いていないので、注文紙を薬局に持って行って買います。

Oral medications are ideal for people who experience symptoms all over the body. Second-generation antihistamines are sold as non-drowsy, but sensitive people still get drowsy, so they need to be careful when driving. They also have side effects such as headaches and dry mouth. First-generation antihistamine, Benadryl. It works very well, but it makes you very sleepy. Some people use it as a sleeping pill. It also has side effects such as dry mouth and eyes, and confusion. When second-generation antihistamines are not effective, some doctors prescribe Benadryl in combination with it. There are no medicines containing psudoephedrine (decongestant) on the shelves, so you have to take an order form to the pharmacy counter to buy it.

点鼻剤は、鼻症状によく効きます。鼻と目は繋がっているので、目の症状にもそれなりに効きます。特にステロイドの点鼻薬はとてもよく効きますが、鼻血が出る方もいます。Afrinなどのdecongestantの点鼻剤は3日間以上使用するとやめた時にリバウンドするので、3日以上は使わないようにしましょう。

Nasal sprays are effective for nasal symptoms. Because the nose and eyes are connected, they are somewhat effective for eye symptoms, too. Corticosteroid nasal sprays are particularly effective, but some people may experience nosebleeds. Decongestant nasal sprays such as Afrin will cause rebound congestion when you keep using for more than three days and stop them, so do not use them for more than three days.

点眼剤は目の症状だけの人に最適です。

Eye drops are best for people who only have eye symptoms.

その他、漢方やホメオパシーの薬がありますが、長くなってしまうので、今回はスキップします。

え？わたしが何を使っているかって？わたし、薬嫌いなんですよー。なるべく使いたくないんですけど、症状が軽い時はホメオパシーのGenexa、症状がひどい時はcetirizineを服用してます。鼻と目だけじゃなくて、皮膚もあちこち痒くなるので錠剤服用です。わたしは残念なことに、眠くならないと謳っているcetirizineでも眠くなります。fexofenadineは仕事にならないほど眠くなってしまいました。そして、loratadineは効かない上に眠くなる。。。あとは、外から帰ってきたらすぐにシャワーして、なるべく外に行かないようにしたり。「点鼻薬は絶対いや！」なのでわたしは使いません。今日久々にWalgreensに行ったら、levocetirizineが売られていたので、試してみようかと思って買ってみました。levocetirizineはcetirizineの光学異性体の一つで、生物的に活性のあるものだけを抽出したものです。より純度が高い？と言っても、効果と副作用にどれだけ影響があるかは疑問ですが（実際に効果に　統計的有意性はなかったはず）。下の子が大学に行ったら、花粉症のシーズンは花粉の飛んでないところに旅行に行ってやる！

What do I take? I hate medicine… I try not to use it, but when my symptoms are mild I take the homeopathic Genexa, and when my symptoms are
severe I take cetirizine. I take an oral med because I get itches in not only my nose and eyes, but also my skin all over. Unfortunately, even cetirizine, which advertises that it is non-drowsy, makes me sleepy. Fexofenadine made me so sleepy that I could not work. And loratadine didn’t work and it just made me sleepy… Other things I do, I shower as soon as I get back from outside and try not to go outside as much as possible. “I absolutely hate nasal sprays!”, so I don’t use them. Today, I went to Walgreens for the first time in a while and they were selling levocetirizine, so I thought I’d give it a try and bought it. Levocetirizine is one of the enantiomers of cetirizine, and it is an extract of only the biologically active ones. Is it purer? I am not sure how much it affects the effectiveness and side effects (I don’t think there was statistical difference in effectiveness). When my youngest child goes to college, I’ll go on a trip to a place where there’s no pollen flying around during allergy season!
